DOS: 6/19/2020: I paid $1,000.00 for a night-guard, extensive cleaning and tooth color for a future crown/ was told insurance wouldn't cover the color. I have the top tier plan with Delta Dental and have never paid out of pocket. My husband also had the same procedure at a dentist down the street Tri-city Dental and paid NOTHING out of pocket!

(Comfort dental charged my credit on account for $850.00 for a "porcelain/ceramic crown CPT: D2740 that was NEVER billed to insurance) Allowable for this CPT code in 2020 for Delta Dental: $345.75.
1) Requested itemized receipt for the $1,000.00 charge and the doctor refused stating the doctor that quoted me no longer works there? Doctors are required to keep medical records for a certain period of time and that receipt should be there. How did they come up with the $1,000.00 charge for the night guard, extensive cleaning and tooth color? There should be a standard price sheet on file.
2) My husband called to speak with billing to obtain our refund and they did NOT verify there was a signed HIPPA on file for this call nor did she verify when I called to discuss this account.
3) Scheduled a meeting with the doctor/owner of comfort dental; Dr. Sessions and provided him my medical records, fee schedule, EOB from insurance and their payment ledger showing the fraudulent charges and explained why I am owed a refund. The doctor proceeded to explain (which I asked his permission to record the conversation) that they bill the patients for everything before insurance? I was never made aware of that nor signed anything agreeing to that considering this payment was for "better quality" service and night guard needed. I proceeded to ask him that if that’s the case where is the credit for the $54.00 dollars my insurance and I paid for the initial visit? Doctor got upset and stated we were being aggressive in his attempt to divert the question.
The doctor then stated "we are trying to recoup the loss here since we have to pay for services on a doctor that's no longer here" and was trying to negotiate and charge me for three times of having to send a crown back to the lab at $110.00 per time sent. I proceeded to tell the doctor that is not correct and showed him in the treatment notes where its states "permanent CRN has been here since December"? Why would you put a temp on me if you had the crown on the two separate visits on 7/8/2020 and 7/13/2020. I also got the temp re-cemented again 12/8/2020 so if you had the tooth "since December" as notated on my 12/8/2020 visit why was that not placed but you're charging me for three lab visits. Is it even legal to charge the patient for the creation of the tooth? I proceeded to ask him for printed proof of the three lab visits on this tooth and again asked for the itemized receipt for the $1,000.00 charge to which he threw up his hands and stated I "was prepared to write a check today" but because I the customer, that's very knowledgeable in medical billing and insurance practices am not permitted to have my full refund provided to me because I proved his billing wrong? I have to pay for backend things that patients should not be charged for and was charged for a tooth I never received and declined to get due to all the issues documented in their notes? A tooth that was never billed to insurance on my second visit.
